Information collected
The demonstration application form can collect contact details, residential address, nationality, date of birth, employer, job position, insurance interest, a photograph and a message. Do not submit real data to a demonstration deployment.
Why information is collected
Information is collected to demonstrate a secure insurance-request workflow and administrator review process. It is not used to make an insurance decision in this fictional project.
Photographs and sensitive details
Uploaded photographs and date-of-birth details require elevated protection. This project limits access to authenticated administrators and uses randomized upload filenames; a real deployment needs further privacy, retention and security review.
Database storage and security
Records are stored in MySQL using prepared statements. Administrator passwords are stored only as password hashes. Production environments should use HTTPS, access controls, backups, logging, least privilege and secure server configuration.
